Your communication preferences are customizable and can be changed at any time.
How can I communicate my medical concerns?
One Chart l Patient offers a messaging service with your healthcare team. You are able to communicate with providers that you have had an appointment with in the past three years or in the upcoming six months.
We ask that you do not use One Chart l Patient to communicate serious medical concerns with your health care team. Please schedule an appointment with your provider to discuss immediate medical concerns or problems. If you are experiencing a medical emergency, call 911 immediately.
Can I pay my bill in One Chart | Patient
Yes, you can view and pay your healthcare bills online quickly and easily with One Chart | Patient.
You will be automatically signed up for paperless billing when you sign up for One Chart | Patient. If you choose to opt out of paperless billing, you may do so by logging into your account, select billing summary, click the blue link that says cancel paperless billing.
One Chart | Patient also allows you to reduce the number of forms and paperwork you need to fill out for each visit and gives you access to all of your itemized bills.

Can I view the health record of another family member or adult?
Yes, you can request proxy access to view the One Chart | Patient account of another individual. The options are to submit electronically through your One Chart | Patient account or to download the printable form from our website, onechartpatient.com. Some forms may require extra documentation of your relationship to the patient. You will be contacted once the form is reviewed.
