Transitional Care

Transitional care specializes in treating individuals who require nursing and rehabilitative services on an inpatient basis but do not require an acute care environment for treatment. The transitional care team utilizes a comprehensive, interdisciplinary approach to care, with physicians, nurses, physical therapists, speech/language pathologists, pharmacists, social workers, dieticians, spiritual care and case managers/discharge planners working together with the patient and family.
